I have had something similar for years already, those spots are nothing actually but does not flare up during rough sex like you mentioned for me

Today when i about to bath, noticed very small red patch on my underwear. Took off and check didi.. noticed flakes around neck of my dick head.. scrap them off and saw spots on tip (similar to the pic above but just a few of them at the tip). Urine no pain.
