Sex After 40: Testosterone Myths, Dodgy Pills and Staying in Good Form

General health information for members. This is not medical advice, so please see a doctor about your own situation. This post does not promote or arrange any services.

Turning 40 doesn't mean your sex life is winding down. The US National Institute on Aging (NIA) says many older couples actually find more satisfaction in their sex lives than when they were younger, with fewer distractions, more time and a better sense of what they want. Your body does change, and knowing how it changes takes away a lot of worry. Here's a plain guide to what's normal, what's hype, and what actually keeps you in good form.

What's normal as you get older

Mayo Clinic describes the usual changes like this:

• Erections may take a bit longer to come and may not be quite as firm.
• You may need more direct touch to get going and stay going.

No need to compare yourself with your 25-year-old self.

The important part: Mayo Clinic says erectile dysfunction (ED) is not a typical part of ageing. Slower doesn't mean broken. And an off night now and then is completely normal at any age. The NIA says ED only becomes something to talk to your doctor about if it happens often.

The "male menopause" myth

You'll often hear that a man's testosterone crashes at 40. The NHS calls "male menopause" an unhelpful and misleading term. Here's what it actually says:

• Testosterone does fall with age, but slowly and steadily, about 1% a year from around age 30 to 40. That gentle drop is unlikely to cause problems by itself.
• A real testosterone deficiency later in life (doctors call it late-onset hypogonadism) does exist. The NHS describes it as uncommon and not a normal part of ageing. It's more likely in men who are obese or have type 2 diabetes.
• Many symptoms people blame on hormones, like low drive, poor erections, tiredness and moodiness, often come from stress, poor sleep, too much alcohol, lack of exercise, money or work worries, or low mood.

So if you're feeling flat, it's worth looking at the whole picture, not just one hormone.

Getting testosterone checked properly

If you do have symptoms, a proper check is simple:

• It takes symptoms plus blood tests. The American Urological Association (AUA) says low testosterone means low levels together with symptoms. Many of those symptoms are vague and can come from other conditions, so a doctor needs to look at both.
• Morning blood tests, usually twice. Cleveland Clinic says testosterone is highest in the morning, so the test usually needs two separate morning samples. Being sick can lower your level for a while, so tell your doctor if you're unwell.
• Lifestyle comes first. Cleveland Clinic says eating well and staying active are the first treatments. Medicine is considered if those don't help after 6 to 12 months, or if symptoms are severe.
• Testosterone therapy isn't for everyone. It's given as gels, injections and other forms under a doctor's care. Cleveland Clinic lists situations where it may not be suitable, such as prostate cancer, a recent heart attack or stroke, or untreated sleep apnoea. If you still want kids, tell your doctor, as there are other options for that situation.

Skip the "natural" booster pills

This is the big one for Singapore bros. The Health Sciences Authority (HSA) keeps finding sex pills and "herbal" boosters spiked with prescription drugs. A few real examples from HSA:

• A product sold as "natural" and "devoid of side effects" contained more than 60 times the usual dose of tadalafil, a prescription ED medicine.
• A "candy" labelled as just ginseng and coffee contained tadalafil at up to 57 times the usual daily dose.
• Fake Cialis and another sex-enhancement product were found to contain a diabetes medicine that dropped men's blood sugar dangerously low and landed them in hospital.

HSA says products from street stalls, unknown websites and dodgy online sellers may be fake, contain the wrong dose, or contain hidden ingredients. Cleveland Clinic also advises avoiding over-the-counter testosterone supplements and seeing a doctor instead.

The good news is that real ED medicines work well and are easy to get properly. A GP can prescribe them after a quick check to make sure they're safe with your heart and any other medicines you take. It's a short, normal chat for them.

Your erection is a health check

Mayo Clinic says ongoing erection trouble can be a sign of a health condition and is a risk factor for heart disease. The same things that affect your blood vessels affect your erections, including high blood pressure, high cholesterol, diabetes, smoking and carrying extra weight. Mayo Clinic says more than half of men between 40 and 70 have some form of ED, so you're in big company.

Look at it this way: sorting out your erections often means sorting out your heart health too. That's a good deal.

Singapore makes the check easy. HealthHub says cardiovascular screening under Healthier SG Screening checks your weight and waist, blood pressure, blood sugar and cholesterol. If you're 40 or above, haven't been diagnosed with these conditions and haven't screened in the last three years, check your eligibility on HealthHub.

Habits that keep you in form

Nothing here is fancy, but it all works together:

• Move more. The NHS recommends at least 150 minutes of moderate activity a week, like brisk walking or cycling, plus some muscle-strengthening work. Even exercising once or twice a week lowers heart disease risk.
• Sleep well. Cleveland Clinic links healthy testosterone to about seven hours a night and to treating sleep apnoea if you snore heavily or wake up tired.
• Go easy on the drinks. The NIA says too much alcohol can cause erection problems.
• Quit smoking if you can. Mayo Clinic says tobacco reduces blood flow and adds to ED over time.
• Check your medicines. The NIA notes some blood pressure medicines, antidepressants and antihistamines can affect sex. Don't stop anything on your own. Ask your doctor whether there's an alternative.
• Look after your head. Stress, anxiety and low mood affect sex at every age. Talking to someone helps.

Safe sex still matters

The NIA puts it simply: age doesn't protect you from STIs. If you're dating again after a divorce, or just enjoying life with new partners, condoms are still your friend and regular STI checks are just routine maintenance. Most STIs are common and treatable or manageable, so there's no shame in getting tested.

When you test, tell the doctor honestly what you did and who did what, so they test the right places.

Where to get help in Singapore

• Your GP or a polyclinic for an erection or hormone check, a proper prescription, or a referral to a urologist or hormone specialist if needed.
• Healthier SG Screening on HealthHub to check your eligibility for subsidised heart-risk screening.
• DSC Clinic (Department of STI Control), 31 Kelantan Lane #01-16. Appointments on 6293 9648.
• Action for AIDS Anonymous Testing Service (ATS) for anonymous HIV and STI testing.
• To report a suspicious sex-enhancement product, HSA's Enforcement Branch is on 6866 3485 or [email protected].
• If it's weighing on you emotionally:
• national mindline 1771 offers 24/7 support. You can also WhatsApp 6669 1771.
• Samaritans of Singapore runs a 24-hour hotline on 1767. You can also WhatsApp CareText on 9151 1767.
• In an emergency, call 995 or go to the nearest A&E.

Life after 40 can be some of the best years for sex and closeness. Keep active, get the basics checked, skip the roadside pills, and enjoy it. Take care, bro.

Haha yes bro! After 40 things just need a bit more warm-up lah. Takes a little longer and needs a bit more touching to get going, and that's totally normal, not a problem. Slow down and enjoy the build-up, sometimes even better leh. Take care bro!
